Thank you for your interest in receiving services for your child with Big Brothers/Big Sisters Association of Medicine Hat & District. Big Brothers/Big Sisters works with the children, youth, parents, schools and mentors to identify and make suitable matches.

The child application package includes the forms listed below. These must be downloaded and returned to our office once you have completed them.

Many of these forms require your signing and dating them in the presence of agency staff.

Once you have returned the package, the child's school will be contacted as well as any other agency professionals involved with your family. After this additional information is received, separate interviews will be set up with the parent/guardian and the child. Both the parent/guardian and the child will then participate in child safety training.

Big Brothers/Big Sisters does continuous family intake in order to bring children one step closer to being matched to an appropriate mentor.
